The Move-in Process
Once you’re ready to move in, our staff is here to help. We’ll do everything we can to make your move as smooth and comfortable as possible. Key steps in the move-in process are listed below. If you have any questions or need assistance during this process, please let us know.
A Community Visit
We strongly recommend that everyone
involved in a resident’s move visit our community prior to move-in.
Visits are a great way to see residents participating in community life
and enjoying a full range of activities, while also providing an
opportunity to meet our vibrant staff members. If your schedule
permits, we would love to have you join us for a meal. Just give us a
day’s notice so our dining room staff can make the necessary
arrangements.
Application Form
Ask your community representative for a Resident Application Form to fill out and return.
Physician’s Assessment
Ask your community representative if a physical exam conducted by a physician is needed prior to move-in. Also ask about any requirement for a test for tuberculosis (TB) or a chest x-ray. Any necessary forms can be provided by your community representative.
The Lease
A signed Residency and Service Agreement, which outlines the terms and included services, along with any remaining Community Fee or Entrance Fee completes the lease agreement process

Copies of the following documentation would be helpful to have on file at the community. Our staff can make copies and return the originals to you.
- Power of Attorney
- Living Will
- Do/Do Not Resuscitate (DNR) Order
- Medicare Cards
- Private Insurance Cards
“Charter Service” Assistance Level Assessment - For Assisted Living and Alzheimer's Care
To ensure the needs of each of resident can be met in Assisted
Living and Alzheimer’s Care, an assessment will be conducted by a
community representative. The assessment will be instrumental in
determining the degree of assistance needed. Assistance needs are
grouped into what Brightview calls “Charter Service” levels. To arrange
an appointment with one of our trained staff who can perform the
assessment and evaluate the appropriate Charter Service level, just
give us a call. They will be happy to make the necessary arrangements.
